Failed to create empty document while running a study.

There are 2 possible solutions:

Solution 1:
a) Document menu > Reports
b) Check the “Disable Report Viewer” Button.

Solution 2:
a) Go to C:\PTW32\lib.
b) Rename “PTWforms.fmt” to “PTWforms_old.fmt.”
c) Restart the software.
